In 1998, Choline Was Ruled an Essential Nutrient. It Still Doesn't Have an RDA.

Choline has a strange resume. In 1998 the Institute of Medicine looked at the evidence, agreed the body cannot reliably make enough of it, and added choline to the short list of essential nutrients. Then it declined to give choline a Recommended Dietary Allowance. There was not enough data to set one, so choline got an Adequate Intake instead, which is the nutrition world's polite way of saying 'our best estimate, and please take it seriously.' More than twenty years later choline is officially essential, carries a Daily Value on the Nutrition Facts label, still has no RDA, and most Americans do not hit even the softer target. That is a lot of contradictions for a molecule most people have never heard of.

Here is what it actually does. Choline is a methyl donor, which means it hands out the chemical tags that switch reactions on and off across your metabolism. It is the raw material for acetylcholine, the neurotransmitter your brain leans on for memory, mood and muscle control. And it builds phosphatidylcholine, the phospholipid that wraps every cell you own. Miss any one of those jobs and you notice. Miss the membrane job and your liver notices first.

The liver part is the headline. Your liver packs fat into little shipping containers called VLDL and mails them out to the rest of the body. Phosphatidylcholine is the tape on the box. Run low on choline and the boxes do not seal, the fat never ships, and it piles up in the liver as steatosis, the first step of what used to be called non-alcoholic fatty liver disease and is now labeled MASLD. This is not a theory built on mouse chow. When researchers fed healthy adults intravenous formulas with plenty of methionine and folate but no choline, they developed measurable liver damage that reversed once choline came back. A 2024 analysis of national US survey data found that higher total choline intake tracked with less liver fibrosis. That is association, not proof, but it points the same way as everything else, which is why the classic lipotropic liver-support formulas pair choline with inositol and methionine.

So if most people fall short, why is the country not full of choline-deficient patients? Because your body cheats. An enzyme called PEMT lets the liver make some choline on its own, and dietary folate and betaine can cover a few of choline's methyl-donor shifts. Frank deficiency in otherwise healthy people is rare. The interesting wrinkle is who cannot cheat as well. That PEMT enzyme is switched on by estrogen, so many premenopausal women synthesize a useful amount internally. Postmenopausal women lose that discount, and a common variant in the PEMT gene turns it down further. Two people eating the identical dinner can have genuinely different choline needs, which is exactly the kind of nuance a single RDA number would have flattened. It also explains why the methylation crew travels together, so a B-complex with folate and B12 is a reasonable partner rather than a rival.

Pregnancy is where the research gets loud. In a controlled 2018 feeding trial, women in their third trimester were assigned either 480 mg of choline a day or 930 mg. The infants in the higher group showed faster information processing speed, measured as reaction time, across their first year. The pregnancy Adequate Intake is 450 mg, and the trial doubled it. That does not make 930 mg a prescription, and none of this is medical advice, but it is a striking result for a nutrient that rarely makes the prenatal shortlist or the average multivitamin, which tends to skimp on choline because the dose is bulky.

Where do you get it. The richest sources are almost aggressively unglamorous: eggs (the yolk specifically), liver, meat, fish and dairy, with cruciferous vegetables, beans, nuts and whole grains filling in. Two eggs put a real dent in the day's number. If eggs are not your thing, supplemental choline exists as choline bitartrate or as phosphatidylcholine. A word on more-is-better, because someone always asks. The Tolerable Upper Intake Level for adults is 3,500 mg a day. Cross it and the side effects are memorable: low blood pressure, and a fishy body odor, because the excess gets converted into a compound that smells exactly like it sounds. Essential does not mean unlimited.
