Fennel has been used in traditional medicine for approximately three thousand years, which raises the question of why the modern clinical trial took quite so long to turn up. When it finally did, in the form of three rather small studies conducted over the past decade, researchers recorded modest effects on post-meal glucose, some reduction in anxiety scores during aromatherapy massage, and a general sense that more work is required. The more work has not yet arrived. One imagines it is stuck in committee.
The fennel seed supplement market, meanwhile, has not waited for the replication studies.
What the Fennel Seed Supplement Glucose Studies Measured, or Didn't
The active compounds in fennel seed supplement products are primarily trans-anethole and fenchone, both of which appear in the essential oil at concentrations that vary depending on which part of the plant one distils, where it was grown, and whether one remembered to label the vials correctly. A 2026 analysis of wild-grown Croatian fennel found that the flowers yielded one point nine five percent essential oil, the mature fruits one point four three percent, the leaves zero point six nine percent, and the stems a rather underwhelming zero point five eight percent. Trans-anethole constituted up to eighty point seven one percent of the fruit oil and forty point nine six percent of the stem oil, which suggests that if one is after anethole, one ought to distil the fruit and leave the stems to someone with lower standards.
The compound itself has been investigated for antioxidant, antimicrobial, and anti-inflammatory effects in laboratory settings, which is to say in dishes and rodents. In human trials examining postprandial glucose, the published work consists of references in broader reviews rather than dedicated controlled studies. The evidence base is thin and the details are scarce. One 2019 systematic review noted that fennel had been administered in capsule or extract form in various herbal intervention studies, but did not provide sample sizes, dosing protocols, or effect magnitudes for glucose outcomes specifically.
The studies exist. The replication does not.
The Aromatherapy Trial That Measured Anxiety in Forty People
A 2026 randomised trial enrolled forty participants and divided them into two groups of twenty. One group received massage with fennel essential oil diluted to one point six percent in sweet almond oil. The other group received massage with sweet almond oil alone, which is to say they were massaged but not aromatically enhanced. The intervention consisted of one session every fifteen days over two months, which works out to four sessions in total, assuming nobody missed an appointment or moved house.

At the end of the trial, the fennel aromatherapy group recorded a mean state anxiety score of thirty-two point four seven, compared to forty-one point seven five in the massage-only group. The difference of nine point two eight was statistically significant. The clinical significance is harder to judge, given that both groups were being massaged regularly by another person for eight weeks, an intervention that one imagines might carry some benefit regardless of what oil is involved. The study did not include a no-massage control, so one cannot say with confidence what the fennel contributed beyond the touch, the time, and the fact that someone was paying attention.
The trial also measured well-being and sleep quality. Eighty-nine percent of the fennel group reported strong well-being at the one-month follow-up, compared to sixty-five percent in the massage-only group. Thirty-two percent reported good sleep, compared to twenty-five percent. The differences were not statistically significant when the mean scores were compared, which is to say the percentages look impressive until one runs the numbers properly, at which point they stop doing that.
The Fenchone Benefits Nobody Has Quantified in Humans
Fenchone, the other major constituent of fennel oil, appeared at concentrations of sixteen to twenty-eight percent in the hydrolates analysed in the Croatian study. Hydrolates are the aromatic water left over after steam distillation, which makes them sound like a byproduct but does not make them useless. The volatile profile of fennel hydrolates turned out to be more complex than the oil itself, particularly in the leaf hydrolate, where twenty-nine distinct compounds were identified. Whether this complexity translates to any meaningful biological activity in humans is not a question the literature has answered. One imagines the compounds are very complex indeed, and very much still in the dish.
The antimicrobial activity of fennel essential oil has been tested against four bacterial strains using the disc diffusion method. The oil demonstrated moderate effectiveness exclusively against Escherichia coli, which the researchers attributed to the high phenylpropanoid content, primarily trans-anethole, and the presence of fenchone. It did nothing to the other three strains, which suggests that fennel is not a broad-spectrum antimicrobial agent and should not be mistaken for one.
What We Do Not Know About Fennel Seed Supplement Effects, a List
A 2024 review on nutraceuticals for paediatric gut-brain interaction disorders noted that significant reductions in colic episodes and crying time were reported in two studies on fennel, administered either as seed oil or as tea, in breast-fed infants. The review did not provide sample sizes, dosing details, or duration for those trials, which makes it difficult to assess the quality of the evidence or to replicate the intervention. It did note that fennel appeared in combination with other herbs in three additional studies, all of which also reported reductions in colic, but separating the fennel effect from the effect of the other ingredients is not possible from the data presented. One assumes the fennel was in there somewhere, doing something or other.
The broader literature on fennel seed supplement use includes references to traditional use in digestive complaints, menstrual discomfort, and respiratory conditions, none of which have been the subject of large, well-controlled human trials. The 2023 review on spices and herbs noted that fennel contains trans-anethole, fenchone, and estragole, and that these compounds show anti-inflammatory, antimicrobial, and antioxidant activities in vitro. It did not quantify those activities in humans, nor did it provide dosing guidance.
This is the recurring theme when one reviews the fennel literature: a great deal of traditional use, a modest amount of laboratory work, and a small number of human trials that have not been replicated.
The supplement market for fennel seed supplement products is nevertheless well established, with products sold as capsules, tinctures, teas, and essential oils. The labelling tends to emphasise digestive support, which is at least consistent with the traditional use, and occasionally mentions antioxidant or wellness benefits, which are harder to substantiate from the published human data. One does wonder whether the market is waiting for the science or simply proceeding without it. The answer, one suspects, is already quite clear.
Three small studies do not constitute a robust evidence base. They constitute a start, a rather preliminary one, and an invitation for someone to fund the follow-up work. Until that work arrives, the fennel seed supplement remains a traditional remedy with a thin file of modern evidence and a great many unanswered questions.
