Fecal microbiota transplantation is when a gastroenterologist takes stool from a screened healthy donor and installs it in your digestive tract. Supplement companies would very much like to sell you a gut microbiome restoration supplement that does the same thing in a capsule.

They cannot.

The gap between what happens in a clinical procedure and what a retail product can accomplish is not a marketing problem. It is biology happening to the marketing.

The difference is a complete ecosystem versus ten bacteria and a pep talk.

What fecal transplants actually transplant

In a clinical trial of recurrent Clostridioides difficile infection, researchers gave nineteen participants lyophilized fecal microbiota transplant. Eighteen other participants got lyophilized sterile fecal filtrate. That second group got the same donor stool with all live bacteria removed.

One worked. One did not.

The study recorded distinct immune reprogramming in the people who received live bacteria: upregulated activation genes in CD4+ and CD8+ T cells, changes in natural killer cell signaling, shifts in mucosal-associated invariant T cells. The sterile filtrate did not produce those changes.

The bacteria were the intervention. The rest was expensive dirt.

A microbiome support supplement does not contain a donor's entire bacterial census. It contains somewhere between one and twenty strains, selected because they survived the manufacturing process and the shelf.

That is not the same installation. That is a window box on a vacant lot.

In studies of graft-versus-host disease, researchers noted that the gut microbiota's collective gene content exceeds the human genome by more than two orders of magnitude. A transplant moves all of that. A probiotic supplement moves the organisms that tolerate freeze-drying and made it through your stomach acid.

You are comparing a municipal water system to a bottle of Evian.

The prebiotic versus probiotic question nobody wants to answer plainly

A probiotic delivers organisms. A prebiotic fiber supplement delivers food for organisms already present.

Neither one rebuilds your entire microbiota from scratch. The industry would prefer you not notice the difference.

In a study of ulcerative colitis, researchers introduced fecal samples from patients and healthy controls into miniature bioreactor arrays. The UC-derived microbiotas showed impaired colonization resistance against C. difficile. Healthy donor microbiotas resisted it. The difference was structural: which organisms were present and in what proportions.

Feeding the UC microbiota more fiber did not turn it into the healthy one. The architecture was different. You cannot renovate a house by mailing it lumber.

If your gut is a failing restaurant, a prebiotic is more napkins.

That does not mean prebiotics do nothing. In mouse models, researchers found that fecal transplants from healthy donors raised serum butyrate levels and reduced tumor burden. Butyrate is a short-chain fatty acid produced when certain bacteria ferment fiber.

A prebiotic can increase butyrate production if you already host the bacteria that make it.

If you do not host them, you are funding a staff you do not have.

The question is not prebiotic vs probiotic which works better. The question is what you are trying to feed and whether it is home.

What researchers actually did versus what you can buy

In a study of colitis-related cognitive impairment, researchers gave mice extracellular vesicles derived from Akkermansia muciniphila. The vesicles reduced colonic inflammation, restored intestinal barrier integrity, suppressed neuroinflammation, and increased hippocampal serotonin receptor expression.

That is one organism's secreted particles doing specific measurable work in a controlled model.

A probiotic supplement for digestive health is not that specific. Your kitchen is not that controlled.

The supplement contains live cultures of maybe Lactobacillus and Bifidobacterium, selected for their ability to survive commercial production. The researchers used a single strain, verified its vesicle cargo with proteomic analysis, and measured its biodistribution to hippocampal neurons.

You are taking ten organisms that made it through a capsule.

These are not the same projects.

In models of pancreatitis, researchers used germ-free mice, fecal transplants from patients, differential antibiotic treatments, and single-bacterial colonization experiments to identify Escherichia coli as a driver of disease severity. The method was isolation and reintroduction in a sterile host.

You cannot do that at home.

The best supplement to restore gut bacteria naturally is still just bacteria in a bottle hoping for the best.

In studies of early childhood malnutrition, researchers noted that microbial alterations included depletion of taxa involved in short-chain fatty acid production and complex carbohydrate fermentation. The proposed intervention was not a gut health restoration powder. It was restoration of microbiota function through developmentally timed interventions in populations where malnutrition remains prevalent.

That is public health infrastructure, not retail.

The thing a capsule cannot deliver

What a fecal microbiota transplant delivers: a complete microbial ecosystem from a screened donor, including organisms that do not survive outside a human gut, installed directly past the gastric barrier, in a medical setting, for a specific indication.

What a gut microbiome restoration supplement delivers: a small number of hardy organisms and some fiber, taken orally, in your own home, with no screening of your baseline microbiota and no measurement of what actually colonizes.

The supplement alternative to medical microbiome procedures does not exist.

If you want to support the bacteria you already have, prebiotics and probiotics may help. Those are different goals. The industry sells them as the same thing because the truth does not fit on a label.

What do prebiotics actually do for digestion? They feed fiber-fermenting bacteria if you have them. What researchers did in IBD models: introduced patient-derived microbiotas into bioreactor arrays and germ-free mice to preserve functional and structural dysbiosis for mechanistic study.

You can buy the fiber. You cannot buy the lab.
