The kidney function supplement you just bought came with promises and zero measurements.
No baseline creatinine clearance. No follow-up kit. No way to know if your glomerular filtration rate moved one decimal place. Just a bottle, a suggested use panel, and the word 'support' doing work a number should be doing.
That's the tell, mijo. If it worked, they'd want to prove it.
What Gets Measured in Real Kidney Studies
Creatinine clearance is how fast your kidneys filter waste. It's calculated from timed urine collection and a blood draw. It's the actual number that tells you whether your renal function is stable, improving or declining.
A 2026 systematic review in Renal Failure examined biomarkers for acute kidney injury in high-risk pediatric populations across 53 studies. Neutrophil gelatinase-associated lipocalin. Kidney injury molecule-1. Cystatin-C. A dozen others.
The review noted serum creatinine remains the diagnostic standard. Not because it's perfect. Because it's what everybody actually measures.
Creatinine clearance derives from it. You collect urine for 24 hours, measure the concentration, do arithmetic involving your blood level and body surface area, you get a filtration rate in milliliters per minute. Normal is 90 to 120. Below 60 is concerning. Below 15 is end-stage.
Most kidney function supplements never touch this number. They sell you on supporting healthy function. They do not sell you on measuring whether that function changed.
The Baseline They Skip: Measuring Nothing, Twice
You cannot verify efficacy without a before and an after.
The industry skips both.
A supplement trial that does not record baseline creatinine clearance cannot report a change in creatinine clearance. Walk the supplement aisle and count how many kidney health markers the marketing mentions versus how many the label actually tells you to measure. Infinity to zero.
The 2026 Renal Failure cohort study of 368 hemodialysis patients in Shanghai compared two dialysate delivery systems. Researchers tracked total protein, albumin, hemoglobin, creatinine and urea reduction over three and a half years. One system was associated with higher albumin and hemoglobin after adjustment.
Creatinine and urea reduction showed no significant difference.

Another 2026 study in the same journal measured serum spermidine in 193 peritoneal dialysis patients. Found it independently associated with vascular calcification. The team built a predictive model that included age, diabetes status, albumin, statin use and peritonitis history. Area under the curve of 0.795 in internal validation.
That's real measurement. The kind you can build on. The kind that changes treatment.
The kind the kidney function supplement you bought does not bother with.
What Research Tracks Versus What Labels Mention
Renal researchers track creatinine, cystatin-C, blood urea nitrogen, estimated glomerular filtration rate, albuminuria, and a growing panel of injury biomarkers that detect damage days before creatinine moves.
A 2026 review in Veterinary and Animal Science on protein digestibility in animals discussed plasma urea nitrogen, blood amino acid profiles, postprandial kinetics as indicators of nitrogen metabolism and renal load. The review noted renal function is a key confounding factor when interpreting urea-based biomarkers. Impaired clearance distorts the signal.
Kidney supplement labels mention supporting healthy function, maintaining wellness, providing antioxidant benefits.
They do not mention measuring any of it.
A third 2026 paper in Renal Failure examined 400 patients with IgA nephropathy. Found the ratio of non-HDL cholesterol to HDL cholesterol was independently associated with tubular atrophy and interstitial fibrosis on biopsy. The highest quartile of this lipid ratio showed significantly worse histologic damage and lower estimated glomerular filtration rate. The researchers built a model combining the lipid ratio with eGFR. Area under the curve of 0.814 for predicting severe tissue damage.
That's what baseline measurement gets you: a model, a cutoff, a way to stratify risk before the damage is done.
The supplement industry's version of baseline measurement is the expiration date printed on the bottom of the bottle.
The Test You Can Buy Versus the Proof You Cannot
You can buy a creatinine clearance test kit. Mail-order, in-office, all of them cost more than the supplement and none of them come bundled with it.
That's the tell.
If the product worked in a way you could measure, the measurement would be part of the offer. It would be the before-and-after photo. The tracking dashboard. The reason you renew.
Instead, the measurement is something you have to go find on your own. Because finding it might produce a number the marketing cannot use.
The pediatric AKI biomarker review noted newer markers like NGAL, KIM-1, and the TIMP-2 times IGFBP-7 product show promise for earlier detection than creatinine. The FDA has approved some of these assays. They are used in ICUs, in surgical recovery, in clinical trials.
They are not used on the back label of a supplement bottle. Because a bottle that measures nothing can promise everything.
Round one: you buy the supplement because the marketing mentioned kidneys and you have kidneys. Round two: you take it for sixty days and feel roughly the same. Round three: you either buy another bottle or you don't, and either way your creatinine clearance was never recorded.
Final score: the industry wins. What it proves: casi nada. Those are different columns.
