Natamycin is an antifungal compound approved for the treatment of severe fungal eye infections, where it is administered topically during corneal surgery, or intravenously in hospital, or occasionally as part of a multi-drug protocol that also involves removing bits of infected tissue with a scalpel. It does not come in eye drops for home use, which is something of an obstacle if one were hoping to support lens transparency from the comfort of one's sitting room. The natamycin eye health supplement market has nevertheless pressed ahead, adding the compound to vision formulas on the argument that an antifungal agent with a long history in cheese preservation might reasonably do something useful for aging eyes, provided one does not ask what the history actually involves.
The history involves corneas being replaced.
What Natamycin Actually Treats, Which Is Fungi Eating Your Cornea
Natamycin has been used as a topical antifungal in fungal keratitis since the nineteen-sixties, a condition in which fungi, typically Fusarium, Aspergillus, or the rather unfortunately named Candida, invade the cornea following an injury. A 2026 case report describes a sixty-six-year-old agricultural worker who was punctured in the eye by a chestnut thorn, which introduced Colletotrichum fructicola, a fungus previously more interested in fruit rot than human vision. After ocular debridement and surgical removal of the foreign body, the patient received topical voriconazole. His corneal clarity improved over three months and his visual acuity rose from zero point three to zero point six, which is presented in the literature as a good outcome.
One imagines it felt rather better than a chestnut thorn.
In cases where fungal keratitis progresses despite antifungal treatment, therapeutic penetrating keratoplasty is performed, a procedure in which the infected cornea is removed and replaced with donor tissue. A 2026 case series of thirteen patients who underwent this surgery, all of whom had smear- or culture-positive fungal keratitis, found that nine of the thirteen, or sixty-nine percent, achieved good graft clarity or became candidates for subsequent optical transplant. The patients received adjunctive topical tacrolimus postoperatively, which the authors suggest may reduce complications. Three patients experienced reinfection and required repeat transplants.
One imagines that was not the preferred outcome for anyone involved.

The mean age of the patients was sixty-two. Seventy-seven percent were male. The best-corrected visual acuity improved by three and a half lines on average, starting from rather poor baselines, as one would expect when the cornea has been destroyed by fungi and subsequently replaced. No oral supplement was involved in any of these cases, which is perhaps worth mentioning before we proceed to the capsules.
The Supplement Industry's Creative Interpretation of Antifungal Ingredients
The term antifungal eye health ingredients now appears in marketing copy for vision formulas containing natamycin, often alongside lutein, zeaxanthin and bilberry extract, none of which have been studied together in a human trial for lens transparency. The theoretical basis appears to be that fungal keratitis damages eyes, natamycin treats fungal keratitis, and therefore natamycin in a capsule might prevent damage that has not occurred. This is the sort of reasoning one arrives at after several glasses of wine and a lengthy discussion about cheese mold.
Which is where natamycin spent most of its commercial life before the supplement industry discovered it.
Natamycin has been used as a food preservative on cheese rinds since nineteen sixty-nine. It prevents mold growth rather effectively and requires no refrigeration, which made it popular in European cheese production and later in processed meats. The compound is considered safe at food-preservative concentrations by every regulatory body that has looked at it, which is reassuring if one is eating cheese. It is less directly relevant if one is hoping to improve eye clarity supplements aging vision outcomes by swallowing the same compound in a gel cap, but relevance has never been the supplement industry's primary concern.
No published study has measured natamycin eye health supplement use against lens opacity, cataract progression, or visual acuity in healthy adults or in adults with age-related vision changes. Not one.
What the Vision Research Actually Measured, Which Was Not Supplements
A 2026 systematic review of corneal transplantation outcomes in infectious keratitis analyzed fourteen studies encompassing fifteen hundred twenty-seven eyes. Therapeutic penetrating keratoplasty accounted for eighty-nine percent of the procedures. Infection control ranged from sixty-nine to one hundred percent, depending on the pathogen. Bacterial keratitis had higher cure rates and better graft clarity than fungal or Acanthamoeba keratitis, which the authors note without surprise, as one would not expect fungi and brain-eating amoebas to behave especially well.
Larger grafts, those exceeding eight millimeters, were associated with increased rejection rates and postoperative complications. The deep anterior lamellar keratoplasty technique, used selectively when the endothelium remained intact, offered higher graft survival and lower immunologic risk, though it was not suitable for full-thickness fungal keratitis.
Visual outcomes were described as limited, reflecting the severity of the disease at presentation, the timing of surgery, and the constraints of postoperative immunomodulation. Early surgical intervention improved anatomical success in severe fungal cases, which is to say it prevented the eye from being lost entirely, rather than restoring it to the clarity one enjoyed at twenty. The studies did not involve oral supplements. They involved replacing corneas that fungi had destroyed, which is a rather different undertaking.
The Distance Between Treatment and Prevention, Which Is Considerable
The best supplement for eye lens clarity, if one is searching for it on the internet, will return a great many results, several of which now include natamycin. What the results will not include is a randomized controlled trial in which healthy adults took natamycin eye health supplement formulas daily and experienced measurable improvement in lens transparency, contrast sensitivity, or any other marker of visual function. The research that exists on natamycin and eyes is exclusively about fungal keratitis serious enough to require surgery, drugs administered in hospital, or both.
One rather hopes not to need it in that context.
A 2023 review of nanomaterial-based ocular anti-inflammatory therapies discusses liposomes, dendrimers, nanoparticles and hydrogels as delivery platforms for drugs targeting conjunctivitis, keratitis, uveitis and scleritis. The review emphasizes biocompatibility, controlled release and targeted delivery, all of which are rather more complex than swallowing a capsule and hoping for the best. The authors note that clinical translation remains impeded by limited predictive capacity of conventional models and by regulatory hurdles, which is a polite way of saying that most of these systems have not yet been tested in humans and the ones that have were not tested for lens clarity in the absence of disease.
Natamycin may yet prove useful in a natural vision support formula, though at present the evidence consists entirely of its efficacy against fungi that are actively eating the cornea. The journey from preventing mold on cheese rinds to supporting aging vision in supplement form is a rather long one, and it appears the industry has made that journey without stopping to conduct a clinical trial along the way.
A brave choice, one supposes.
