White peony root is being marketed for menopause hot flash relief, nearly always paired with licorice in what traditional Chinese medicine calls a fixed herbal combination, and I am here to tell you what the science actually shows before you hand over thirty dollars for dried plant material based on a study where twenty women rubbed cream on their faces for four weeks and then filled out a questionnaire.

Twenty women. Four weeks. Face cream. With four other herbs mixed in.

Let me walk you through this real carefully, cher, because the distance between what got studied and what is being sold to you right now is roughly the size of Lake Pontchartrain, and the supplement industry is standing on one side waving a banner that says peony root menopause while the actual research is on the other side whispering something way more boring into a very small microphone.

What Twenty Women and a Jar of Expensive Face Cream Actually Proved

Researchers in 2024 recruited twenty postmenopausal women experiencing vasomotor symptoms, which is the medical term for hot flashes and the sweating that comes with them, and gave them a topical cream containing extracts of five traditional Korean herbs: black cohosh, white peony root, Phellodendron bark, Chinese rhubarb and skullcap. The women applied the cream to their faces twice daily for four weeks, and at the end of the trial, visual analog scale assessments showed substantial reductions in facial flushing severity and associated sweating. Skin redness, hemoglobin concentration and moisture content all improved measurably.

That is legitimately good news for those twenty women, and if you are selling a five-herb topical cream for menopausal skin symptoms, you have one small but real human trial to point at and feel reasonably proud of yourself.

If you are selling a white peony root supplement in capsule form as a standalone ingredient, you have got absolutely nothing here, because this study does not isolate peony, does not test ingestion, and does not tell you which of the fourteen identified bioactive compounds in that mixture did the actual work.

Fourteen compounds. Five herbs. Twenty people. Four weeks. Rubbed on faces.

You cannot pull one root out of that pile and declare it the hero. That is like listening to a brass band at a second line, hearing one clean trumpet solo, and deciding the trombone player was just back there for the exercise.

The Peony and Licorice Pair: Two Thousand Years of Doing Something Together

To be fair to the white peony supplement crowd, they are not inventing this pairing out of thin air and a marketing meeting. A 2026 review analyzed two hundred and eight classical Chinese medicine formulas and found that white peony root, known in TCM as Paeoniae Radix Alba, is one of the most frequently combined herbs in the entire catalog, and its most common dance partner is licorice root. The herbal pair shows up constantly in formulas for menstrual disorders, gynecological complaints, pain relief and what traditional practice calls smoothing the liver and circulating blood, which I assume made more sense before we had imaging.

The review identified thirty-six different herbal pairs containing peony that appear at least fifteen times across historical texts, and keyword analysis of the corresponding modern literature shows that menstrual disorders is the single most common clinical application, which at least suggests that generations of TCM practitioners were aiming this combination at something real and observing results consistent enough to keep writing it down.

That is worth knowing. It is also not a clinical trial.

Traditional use tells you where to look. It does not tell you that the thing works, or how much to take, or whether it works better than a placebo, or whether the combo works because of peony, because of licorice, or because of some chemical handshake between the two that neither one pulls off alone.

Processing Changes Everything, Assuming You Even Know Which Version You Got

Here is another problem nobody is discussing on the label, and I need you to stay with me because this one gets into the weeds but it matters: white peony root is not one product. A 2026 chromatography study analyzed fifteen batches of Paeonia lactiflora root processed to varying degrees by stir-baking, which is the traditional method of heating and stirring the root in a pan to different endpoints, and found that the chemical profile changes dramatically depending on whether you are looking at raw root, stir-baked root, charred root or carbonized root.

Paeoniflorin and benzoylpaeoniflorin, two of the compounds most often cited as the active constituents, decrease progressively as processing intensity increases. Gallic acid, which barely registers in raw root, spikes hard in charred preparations. Albiflorin and a compound with the absolutely ridiculous name of 1,2,3,4,6-O-pentagalloylglucose both peak in the lightly stir-baked version and then drop off a cliff.

The study identified gallic acid as a key discriminatory marker using partial least squares discriminant analysis, which is a statistical method for figuring out which chemical is doing the most to separate one batch from another, and they confirmed through simulated processing that paeoniflorin can decompose into benzoic acid while the pentagalloyl compound degrades into gallic acid.

Translation: the supplement you bought might contain mostly paeoniflorin, or mostly gallic acid, or a bunch of benzoic acid, or some ratio of all three, depending entirely on how hot someone got the root before they ground it up and put it in a capsule, and your bottle does not say which one you got, because the label just says peony root and maybe has a picture of a flower on it.

I realize I am now yelling about chromatography data on a plant root cooked in a wok, but this is the part where I point out that standardization matters, baby. If the active constituent changes depending on whether the root got lightly toasted or turned into actual charcoal, you cannot dose the product, you cannot replicate a study, and you definitely cannot make a clean claim about what peony root does, because peony root is at least five different chemical profiles pretending to be one ingredient.

What We Do Not Have, Which Turns Out to Be Most of It

We do not have a placebo-controlled trial of white peony root extract given orally to menopausal women measuring hot flash frequency over twelve weeks. We do not have a dose-response study. We do not have safety data in populations taking it long-term. We do not have head-to-head comparisons against black cohosh or soy isoflavones or against the extremely popular intervention of doing absolutely nothing and waiting it out with a fan and some patience.

What we have is a four-week trial of a peony root and licorice combination mixed with three other herbs, applied to skin, in twenty women, showing improvement in flushing severity by self-report. That study explicitly framed the topical route as an advantage because it avoids systemic exposure and the potential side effects of ingested phytoestrogens, which means the researchers themselves were not trying to make a case for swallowing peony. They were trying to make a case for rubbing it on your cheeks.

The supplement industry heard "reduces hot flashes" and sprinted straight past "topical," "twenty people," "four other herbs in the mix," and "we have no idea which part did the work."

Look, I am not saying white peony root does nothing. I am saying we do not know what it does, in what form, at what dose, by what mechanism, or whether it does anything at all when you pull it away from licorice and take it orally instead of rubbing it on your face like moisturizer. Traditional use is a hypothesis. A twenty-person trial of a multi-herb topical cream is a pilot study that should lead to a bigger trial. Neither one of those things is evidence strong enough to support the claims currently being made on product pages with shopping carts attached.

If you want to try it anyway, fine. People have been combining peony and licorice for two thousand years without obvious disaster, which is worth something. But go in knowing that the research base is thin, the product quality is all over the map, and the distance between what got studied and what is in the bottle is about the size of the whole Mississippi River, and nobody is building you a bridge across it.
