Soy Isoflavones and Hot Flashes: A Small, Repeatable Effect

Soy isoflavones are plant compounds found in soybeans. They are shaped a little like estrogen, which is the reason researchers keep pointing them at menopause. The trials exist, the effect is real, and the effect is small. Here is what the numbers actually say.

What they are

Isoflavones are a group of phytoestrogens (mainly genistein, daidzein, and glycitein) concentrated in soybeans. Phytoestrogens are plant molecules that can bind estrogen receptors, weakly. Weakly is the operative word. Supplements deliver them extracted or synthesized and standardized to a dose in milligrams, and the menopause trials tended to cluster around a median near 54 mg a day. Some products sell as isolated soy isoflavones, others sit inside a broader phytoestrogen or menopause formula.

Hot flash frequency and severity

Taku and colleagues (2012, Menopause) pooled randomized controlled trials: 19 reviewed, 17 combined. The dose ran at a median of 54 mg of isoflavones (aglycone equivalents) for six weeks to twelve months. In the pooled data, hot flash frequency was 20.6% lower than placebo (95% CI, -28.4 to -12.9), and severity was 26.2% lower (95% CI, -42.2 to -10.2). The authors added a detail worth keeping: supplements providing more genistein (over 18.8 mg) were more than twice as potent at lowering frequency as lower-genistein ones. Dose and form were not interchangeable.

The larger pool of women

Franco and colleagues (2016, JAMA) ran a wider sweep: 62 studies and 6,653 women. Phytoestrogen use was associated with about 1.3 fewer hot flashes a day (95% CI, -2.02 to -0.61) and a lower vaginal dryness score. Night sweats did not move significantly. Soy isoflavones on their own tracked with roughly 0.8 fewer hot flashes a day. The same authors flagged that 74% of the included trials carried a high risk of bias in three or more areas, which is the sort of caveat that belongs in the same sentence as the result.

A third reading

Chen and colleagues (2015, Climacteric) looked at 15 randomized trials. On the Kupperman index, a broad menopause symptom score, phytoestrogens showed no significant advantage over placebo. On hot flash frequency specifically, they did, a significant reduction (p less than 0.005). Side effects were no different from placebo. The pattern is narrow: the signal shows up for hot flash counts, not for every menopause measure you can name.

Bone, briefly

Kanadys and colleagues (2021, International Journal of Environmental Research and Public Health) pooled trials measuring bone turnover markers in postmenopausal women: 1,114 taking isoflavones (mean 98.2 mg a day) and 1,081 on placebo, for three to twenty-four months. Markers of bone formation trended upward and markers of breakdown trended downward, but none of the changes reached statistical significance. The authors described a possible benefit that needs larger studies. Interesting, not settled.

The honest summary

Across four reviews, soy isoflavones are associated with fewer and less severe hot flashes, by a modest margin, over weeks to months. They are not equally convincing for every symptom (night sweats and broad symptom scores are less impressed), and the bone story is a trend rather than a conclusion. Genistein content and dose appear to matter. None of this makes soy isoflavones a drug, and none of these trials tested them as one.
