Stannous Fluoride Toothpaste Makes Your Gums Stop Bleeding By Adding Tin

Somebody decided to put tin in toothpaste, and I need you to understand how insane that sounds when you say it out loud. Tin. The stuff they make cans out of. The material that has been keeping your soup shelf-stable since the Roosevelt administration, and I am talking about the first one, the one with the mustache and the opinions about national parks.

And here is the part that makes me unreasonably angry: it works better than regular fluoride.

Researchers ran an eight-week trial on thirty-nine adults with gingivitis, split into two groups they diplomatically called high bleeders and low bleeders, which is the kind of clinical language that makes you grateful you were not in the room when they were handing out assignments. The high bleeders brushed twice daily with stannous fluoride toothpaste, which is tin fluoride wearing a name tag that went to medical school, and the results were not subtle.

Bleeding dropped. Inflammation dropped. Eighty proteins associated with tissue damage and oxidative stress dropped, including a bunch with names like matrix metalloproteinases, which I assume are the proteins that show up when your gums have had enough of your nonsense and start sending strongly worded chemical complaints to upper management.

What Stannous Fluoride Actually Does That Sodium Fluoride Just Watches

Regular sodium fluoride prevents cavities. It has been doing that job since 1945 and nobody is arguing with its resume, the resume is fine, the resume has references. But for gum health, stannous fluoride vs regular fluoride is not even a fair fight, because the tin version kills bacteria that sodium fluoride just stands next to like a guy at a party who does not want to get involved.

The trial measured the oral microbiome before and after treatment, which means they swabbed people's mouths and counted what was living in there, and what they found was a full demographic shift. Two bacteria in particular, Porphyromonas and Fusobacterium, dropped hard after eight weeks of stannous fluoride use. These are not neutral residents minding their own business. They thrive in inflamed gum tissue, they contribute to bleeding, and they were doing great in the high bleeders at baseline, just throwing a party in there while your gums bled into the sink.

Meanwhile, helpful bacteria like Rothia and Haemophilus increased. The researchers describe this as restoring microbial balance. I describe it as evicting the loud neighbors at two in the morning and replacing them with the kind of people who bring you soup when you have the flu.

I realize that sounds like I am accusing your mouth of running a gentrification project.

I am.

The Protein Situation Got Completely Out Of Hand

Eighty proteins in the participants' saliva changed significantly after treatment. Eighty. The study did not just check whether gums stopped bleeding, they measured what was happening at the molecular level while the bleeding stopped, and what they found was that stannous fluoride was shutting down an entire inflammatory response that had apparently been running a fever for months.

Pro-inflammatory cytokines dropped. Alarmin keratins, which are proteins that scream "something is wrong" when tissue gets damaged, dropped. Matrix metalloproteinases, the enzymes that break down collagen when your gums are under sustained attack, also dropped, which suggests that the best toothpaste for gum sensitivity might be the one that convinces your own immune system to stop eating your gum tissue like it is settling a personal grudge.

And look, I am aware I am getting worked up about protein expression in human spit. Stay with me. Twenty-nine of those eighty proteins overlapped with disease biomarkers in something called the plasma proteome atlas, which tracks proteins associated with systemic health problems, meaning this is not just about whether you bleed on your floss anymore. The researchers are suggesting, very carefully, in the way scientists suggest things when they do not want to get yelled at, that improving oral health with stannous fluoride might reduce systemic inflammation.

That is the kind of claim that makes you sit up and wonder if we have been thinking about toothpaste wrong this entire time, like we have been using a firehose to water a plant and just figured out it also puts out fires.

Why Tin, Of All The Elements On The Periodic Table

Stannous fluoride has been around since the 1950s. It is not new. It is older than your parents' record collection. But it fell out of favor for a while because early formulations stained teeth, tasted like you were licking a battery terminal in a rainstorm, and were generally unpleasant enough that people went back to sodium fluoride and accepted the trade-off of gums that bled a little.

Modern formulations fixed the staining problem, mostly, and the taste problem, sort of, but a lot of people still do not know the difference exists or why it would matter. A systematic review of oral care guidelines published in 2025 noted that fluoride toothpaste works, full stop, and did not get into the weeds about which kind of fluoride, possibly because the evidence for stannous fluoride's superiority in gum health is relatively recent and guidelines update at the speed of continental drift.

But here is what we know now: tin ions are antibacterial in a way that sodium ions are not. Stannous fluoride does not just sit on your teeth preventing cavities like a very polite houseguest. It actively kills the bacteria that cause gum inflammation, and it does it better than the standard stuff, at least according to this trial and the small mountain of salivary protein data they collected to prove it.

Does This Mean You Throw Out Your Current Toothpaste And Start Over

I do not know, cher. That is between you and your gums and possibly your dentist if your dentist has opinions.

What I do know is that if your gums bleed when you floss, or if a dentist has used the word gingivitis in your presence in the last calendar year, switching to a fluoride toothpaste that addresses gum bleeding is worth considering, and the research says stannous fluoride does more than sodium fluoride on that front. The trial ran for eight weeks. Participants brushed twice a day. Bleeding and inflammation dropped. Harmful bacteria dropped. Tissue integrity improved, as measured by reduced collagen breakdown, which is a fancy way of saying your gums stopped falling apart at the seams.

None of that is a cure, none of it replaces flossing or checkups, and if you have actual periodontal disease, the kind where a hygienist uses words like "pockets" and "bone loss," a dentist is the move, not a different tube of paste. But for early-stage gingivitis, the kind where your gums are mad at you but have not yet hired a lawyer, this is one of the few interventions with recent controlled-trial evidence showing it actually moves the needle instead of just sitting there looking helpful.

And yes, it is weird that the active ingredient is tin. It is weird that the same element keeping your canned tomatoes from turning into a biological weapon also keeps your gums from bleeding all over your dental floss like a crime scene. I do not make the rules. I just read the studies and get annoyed that they are right.
